This document is attached to each application we issue.
Once your application is complete you will need to gather and bring the following to your meeting with the Trustee or case worker:
A. Proof of income for ALL household members for the last 30 days, check stubs, direct deposits, unemployment benefit records, SSI & SSD records, child support, etc.
B. Proof of food stamps - you should have a letter from them stating amount and effective date.
C. ALL of your utility bills.
D. Your lease, rental agreement or mortgage payment statement.
E. Bank statements with detailed credits, debits and balances.
F. Any court or medical records that pertain to your current circumstances.
When you have all of this assembled contact us for an appointment. Call 260-244-0045 or email us at trustee@columbiatownship.net
If you do not have any one of the above, call and talk to the Trustee about what you do not have.
Bring all of this to the appointment.
